Family Questionnaire

When entering school, parents need to fill in a family questionnaire, which include imformation like the home address of the students, the names of the family members, the work places of the parents, or the name of the schools where their siblings are attending. Parents may write down the things they would like to inform the teacher about. Every spring parents fill in a family questionnaire and submit it to the school.
